At first glance, these items appear to be simple quest leftovers used to unlock Purified Slots for Divine Gifts via Hadriel. However, players quickly noticed something strange: Unique Purified Essences keep dropping, even after all Purified Slots are unlocked.

What Are Unique Purified Essences in Diablo 4?

As you fight Lesser Evils across Sanctuary in Season 11, you may receive special drops known as Unique Purified Essences. Each Essence is tied to one of the four Lesser Evils:

Andariel
Duriel
Azmodan
Belial

These Diablo 4 Items are distinct from standard Purified Essences and cannot be consumed at Hadriel. Instead, they correspond to mysterious altars hidden throughout the world-a secret system that Blizzard has not officially explained in-game.

What to Do With Unique Purified Essences

Each Unique Purified Essence must be taken to a specific altar located in the Sanctuary. When you interact with the correct altar using the matching Essence, the altar ignites with hellfire-but nothing else appears to happen immediately.

This led many players to believe the system was unfinished. In reality, you must activate all four altars to trigger the secret encounter.
Once all altars are lit, a red portal opens, summoning a brutal multi-phase fight involving all four Lesser Evils.

How to Get Unique Purified Essences

You can obtain Unique Purified Essences by defeating the following bosses in Season 11:

Andariel - Kurast Undercity
Azmodan - Hawezar
Belial - The Pit of the Artificer
Duriel - Helltides
